Job Description:
A company is looking for a Member Care Navigator to assist low
complexity members in navigating health needs and social
resources.
Key Responsibilities
Engage with members to explain program expectations and goals
Conduct assessments and collaborate with RN Care Managers on care
plans
Follow up on care coordination needs and provide non-clinical
education
Required Qualifications and Education
Comfortable using technology for remote member support
Understanding of scheduling and electronic health record
systems
Proficient in collecting and documenting member data
Knowledge of Motivational Interviewing and Trauma Informed Care
principles
Adaptable to fast-paced changes and shifting priorities
