Job Description:
A company is looking for a Care Manager, RN.
Key Responsibilities
Develop, oversee, and revise patient plans of care in collaboration
with field staff
Manage health care costs by influencing patient care decisions and
adhering to utilization management guidelines
Participate in interdisciplinary team meetings and ensure
compliance with regulatory standards
Required Qualifications
Registered Nurse licensure required
Two years of home care field experience preferred
One year of clinical review experience preferred
Two years of case management and/or utilization management
experience preferred
Knowledge of Medicare, Medicaid, and compliance guidelines for
patient care
