Job Description:
A company is looking for a Complex Care Manager - High Risk
Diabetes.
Key Responsibilities
Advocates and educates members on services and self-management
techniques
Conducts assessments and develops care plans in alignment with
treatment goals
Coordinates care with healthcare providers and documents all member
interactions
Required Qualifications
NYS RN or relevant mental health license (LCSW, LMSW, LMFT, LMHC,
LPC, licensed psychologist)
Experience managing large caseloads in a fast-paced environment
Knowledge of community health practices for frail adults and
cognitive impaired seniors
Remote work experience preferred
Fluency in Spanish, Korean, Mandarin, or Cantonese is a plus
